The idea that hormone fluctuation drives eating urges comes from 196 regularly cycling women aged 16–22, where emotional eating tracked an estradiol-by-progesterone interaction across the cycle; that was never studied in perimenopause, so applying it here is inference, not proof.4 The popular "menopause spikes your hunger hormones" line is weaker still: in a cross-sectional study of postmenopausal women that did not even measure hunger, ghrelin's link to estradiol disappeared after adjusting for BMI, though leptin still tracked it.5 And "food noise," intrusive food thoughts apart from physical hunger, is a real but young idea, formalized in a 2023 model with a validated questionnaire only arriving in 2025, tested in adults with obesity rather than perimenopause.3, 8
So the metabolic story is well-established, while the cravings story is thinner and confounded, especially by disrupted sleep, and no primary study shows perimenopause causes more cravings.1 What you feel is real; the useful response is to gather your own data.
Does self-tracking help?
Modestly, and generally. A systematic review of 59 behavioural weight-loss trials found most that used dietary self-monitoring showed significant weight loss versus comparison groups.6 But self-monitoring was bundled inside larger programs with varied comparison groups, so its solo effect is unclear, and none of this was perimenopause-specific.6 Treat this log as a self-awareness aid to inform a clinician conversation, not a validated instrument or a treatment.

A prose version works too: each evening, write two or three sentences on how you slept, when hunger or cravings peaked, what was going on, and whether the pull felt like body hunger or mental noise.
How to read your log
After 14 days, look for clustering rather than single bad days.
- Sleep. Mark nights under about six hours or rated poor. Do the next day's hunger and cravings run higher? Disrupted sleep is common in perimenopause and is a frequent confounder in cravings research, so this is often the clearest signal.
- Cycle phase. If you are still cycling, mark the one to two weeks before an expected period. Cravings clustering there echo the luteal-phase pattern seen in younger cycling women; with erratic perimenopausal cycles this may be messy.
- Skipped meals. Look at mornings you ate very little. Does hunger or a strong evening urge spike later? That is a behavioral loop you can adjust.
- Hunger versus food noise. Physical hunger tends to build gradually and ease after eating; food noise is a mental pull, often tied to stress, boredom, alcohol, or seeing food, that can persist after a meal.
