A few things this makes plain:1
- The widely quoted "−4.5% / −1.4 kg" figure is a per-protocol result for the B420-plus-fiber arm, not for B420 alone and not the primary result. Per-protocol analysis drops people who quit or did not take the product as directed, which can tilt results in a treatment's favor. The overall per-protocol test across arms was not significant (P=0.095).
- B420 by itself was not significant on fat mass (P=0.28).
- The dramatic "−4.0%, P=0.002" is a post-hoc analysis comparing the two arms that contained B420 against the two that did not. It is not the "B420 alone" arm, and a post-hoc comparison does not carry the weight of a pre-specified endpoint.
How funding affects confidence

The mechanism tier: promising in mice, not proof in people
In high-fat-diet mice, B420 reduced fat accumulation and improved gut-barrier function, which points to a plausible pathway.2 Keep the tier straight: this is animal and mechanistic evidence, and the 2020 review's own authors wrote that the pathways are not fully understood and that further clinical studies are needed.2 Results in mice do not predict results in people.
Probiotics as a class: real but small
