Food noise describes persistent food thoughts that feel hard to switch off. What food noise is, and isn't

A 2025 expert-perspective paper describes food noise as persistent thoughts about food that a person perceives as unwanted or distressing and that may cause social, mental, or physical harm; the same paper is candid that a formal clinical definition and an agreed measurement method are still lacking.1 So be careful with the language: food noise is not in the DSM, the ICD, or any FDA drug label. No one can currently "test positive" for it. It is a useful word for a real, distressing experience, backed so far by expert opinion rather than a validated diagnostic construct.1

Where the term came from

Food noise overlaps heavily with a construct that already had decades of research behind it: food cue reactivity, meaning how strongly you respond to the sight, smell, or thought of food. A few caveats on this table:

  • The FNQ is the strongest food-noise-specific tool. Its validation showed high internal consistency (Cronbach's alpha 0.93) and good 7-day retest reliability (r=0.79 in a subsample), which makes it the most rigorously validated option.3 But it was validated in a general adult sample, not yet as a way to track response to a GLP-1 drug, and no published FNQ score currently defines "clinically significant" food noise.3
  • The RAID-FN deserves a flag. It appeared alongside the 2025 definition paper and is co-branded with a company that sells the very medications people credit with quieting food noise, and it has not undergone independent peer-reviewed validation.1 Treat it as an industry instrument, not a neutral standard.
  • CoEQ and FCQ-T are older and better established, but they measure cravings and control of eating, which relate to but are not the same as intrusive food thoughts.

The construct is still being argued over. A published Response to the FNQ paper pushes back on how distinct food noise is from existing measures, while a 2026 commentary argues it is a real phenomenon deserving clinical recognition.4, 5 None of these is yet an independently replicated, treatment-outcome gold standard.

How to track your own food noise

You do not need a clinic or a product to keep tabs on this. Borrow the FNQ's five dimensions and add a simple daily log. This will not diagnose anything, but it gives you and your clinician real data instead of a vague "it's better."

Rate five food-thought dimensions over the past two weeks:3

1. How persistent the thoughts feel.
2. How difficult they feel to control.
3. How much time they occupy.
4. How much they affect daily life.
5. How often they distract from other tasks.

Then keep a short daily journal. Once or twice a day, give the food thoughts a 0–10 intensity score. Note the time, recent meals, stress, sleep, and any food cues. Add what you did and whether the thoughts passed.

After two or three weeks, look for patterns. Rising scores around poor sleep or skipped meals are more actionable than a single bad afternoon. Bring the log to your clinician, especially if the noise is causing real distress, and interpret it together. Do not start, stop, or change any medication based on it.

The GLP-1 link, tiered by evidence

The popular story runs ahead of the data.

Tier 1: Randomized controlled trial (strongest). In the STEP 5 trial, semaglutide 2.4 mg improved control of eating versus placebo over 104 weeks, measured with the validated 19-item CoEQ; Craving Control and Craving for Savory improved significantly at weeks 20, 52, and 104 (p<0.01).7 The caveats: this came from a subgroup who completed the CoEQ (semaglutide n=88, placebo n=86, of 304 randomized), it was funded by the drug's maker, and it measured cravings and control of eating, not "food noise" by name.7

Tier 2: Retrospective industry survey (weak). The widely quoted reductions come from the INFORM survey of US adults on semaglutide, which reported large drops in food-related thoughts and daily-life disruption after starting treatment.6 Treat those figures with real caution: the survey was sponsored by the drug's maker and built on retrospective self-report, so people already on the drug recalled how they used to feel (recall bias) and only continuing users were surveyed (selection bias).6 It is a recalled within-person change, not a randomized comparison.

So GLP-1 medications reduce cravings and appetite in randomized trials, and users commonly report less food noise in surveys, but those are two claims at two evidence levels. There is no longitudinal study yet validating food-noise scales as GLP-1 treatment outcomes.
