Probiotic and Berberine Combination Products, Compared Side by Side

Two capsules beside an unmarked bottle illustrating a combination supplement

A handful of products put a probiotic or Akkermansia ingredient and berberine (or the better-absorbed dihydroberberine) in one daily dose: WonderBiotics Probiotics for Weight Management, Florajen Menopause Probiotic, FOLONA Akkermansia with Berberine, DietWorks GLP-1 Companion, Toniiq GLP-Core 2000, and PLZ Control My Hunger. They differ in strain naming, berberine form, dose disclosure, and price.

Which Products Combine a Probiotic With Berberine?

The products below combine a probiotic or Akkermansia ingredient with berberine HCl or dihydroberberine (DHB). Compare the daily serving, strain identity, and other active ingredients rather than treating all combinations as interchangeable. Approximate daily costs use the specified purchase price and directed serving, before tax, shipping, or promotions; subscription prices are shown separately.

Product Probiotic or Akkermansia (strain named?) Berberine form and amount Ingredient amounts on label Daily cost
WonderBiotics Probiotics for Weight Management1 8 strains, all named with strain codes (B420, HN019, NCFM, GG, Ls-33, Ll-23, Lpc-37, Lp-116); 20 billion CFU total Dihydroberberine 100 mg, plus lemon fruit extract Key amounts listed: B420 10 billion, HN019 2 billion, NCFM 1 billion CFU; DHB 100 mg $1.85 one-time; $1.39 to $1.57 on subscription
Florajen Menopause Probiotic2 Bacillus subtilis DE111, 1 billion CFU Berberine HCl, 100 mg Fully listed $0.50
FOLONA Akkermansia GLP-1 Probiotic with Berberine3 Akkermansia muciniphila AH39, 3 billion AFU (listing describes it as pasteurized) Berberine HCl 97%, 165 mg Fully listed $0.42
DietWorks GLP-1 Companion4 Bacillus subtilis DE111, 1 billion CFU Dihydroberberine (GlucoVantage), 100 mg Mostly listed; three blends give a total only $0.70
Toniiq GLP-Core 20005 Akkermansia muciniphila AH39, pasteurized, 5 billion TFU Dihydroberberine (GlucoVantage), 200 mg Fully listed $1.22
PLZ Control My Hunger6 Akkermansia muciniphila, 5 billion AFU; no strain code given Berberine HCl, 1,000 mg Fully listed $2.67 one-time; $2.13 on subscription

Two terms in that table are easy to misread. AFU and TFU are counting units some brands use for Akkermansia instead of CFU, and they are not interchangeable with CFU. "Pasteurized" means the bacteria have been heat-inactivated, so that ingredient is no longer a live probiotic; brands often market it as a postbiotic.

How Each Combination Formula Is Built

WonderBiotics Probiotics for Weight Management. Our formula pairs eight named probiotic strains, led by Bifidobacterium animalis subsp. lactis B420, with dihydroberberine and a lemon fruit extract standardized to 70% eriocitrin, in one capsule taken after a meal.1 It has the most named strains in this table, each listed with its strain code, and it is the only one that adds an eriocitrin-standardized citrus extract to the berberine side. The label lists the amounts that matter most: B420 at 10 billion CFU, the same daily amount used in the B420 trial, HN019 at 2 billion, NCFM at 1 billion, and 100 mg of dihydroberberine.1

Florajen Menopause Probiotic. A menopause formula from a long-running probiotic brand. One capsule delivers 100 mg berberine HCl, 1 billion CFU of the spore-forming strain DE111, 106 mg NovaSoy soy isoflavones, and 40 mg black cohosh root powder.2 Every amount is printed on the label, and it is the lowest-cost option here that uses a named live strain.

FOLONA Akkermansia GLP-1 Probiotic with Berberine. Each capsule lists 3 billion AFU of Akkermansia muciniphila strain AH39, 165 mg berberine HCl from Indian barberry, and 150 mg oat beta-glucan on the Amazon label (FOLONA's own product page lists 160 mg).3 FOLONA states that the Akkermansia is pasteurized rather than live, so it is an inactivated microbial ingredient paired with berberine.

DietWorks GLP-1 Companion. A two-tablet serving with 100 mg dihydroberberine as GlucoVantage and 1 billion CFU of DE111, plus vitamins, minerals, a 2 g amino acid blend, colostrum, CoQ10, and two 100 mg blends (one "pro collagen," one herbal digestion blend that includes Chinese rhubarb root).4 Berberine and probiotic make up a small part of a broad multi-ingredient tablet.

Toniiq GLP-Core 2000. Four capsules a day provide 200 mg dihydroberberine as GlucoVantage, 5 billion TFU of pasteurized Akkermansia muciniphila AH39, gymnema, Ceylon cinnamon, quercetin, inulin, and chromium.5 Like FOLONA, the Akkermansia here is heat-inactivated.

PLZ Control My Hunger. Two capsules a day deliver 1,000 mg berberine HCl, 5 billion AFU of Akkermansia muciniphila, 200 mg 5-HTP, 88 mg saffron extract, and 200 mcg chromium picolinate, with every amount disclosed.6 It carries the highest berberine dose in this group. The page does not give a strain code for the Akkermansia or say whether it is live or pasteurized.

Does Pendulum Contain Berberine?

Pendulum's current shop page (Akkermansia, Glucose Control, Metabolic Daily, Polyphenol Booster, and Gut Fuel) shows no product containing berberine or dihydroberberine, and the ingredient lists on its Akkermansia and Glucose Control pages contain none.7 Pendulum focuses on next-generation strains such as Akkermansia muciniphila, Clostridium butyricum, and Bifidobacterium infantis, so it belongs in a different category from the combination formulas above.

You will also find dozens of Amazon listings with "GLP-1," "Akkermansia," and "berberine" in the title. Some blend in a long list of other ingredients. Before you add one to your shortlist, open the Supplement Facts image and confirm that both a probiotic and a berberine form appear with amounts.

Why Combine a Probiotic and Berberine in One Formula?

Standard berberine is poorly absorbed, and preclinical research shows gut bacteria help determine how much of it gets in. In animal work, gut bacteria convert berberine into dihydroberberine using nitroreductase enzymes; the DHB showed roughly fivefold greater intestinal absorption, then turned back into berberine in the intestinal tissue.8 When the researchers cut gut bacteria with antibiotics in diabetic mice, less berberine reached the blood and its glucose- and lipid-lowering effect weakened.8 These findings come from rodents, cells, and lab cultures. The human question is whether adding a probiotic changes what berberine does.

The largest human test of the pairing is the PREMOTE trial. Researchers in China randomized 409 adults with newly diagnosed type 2 diabetes, none on diabetes drugs, to berberine (1.2 g a day), a probiotic powder, both, or placebo for 12 weeks, after a one-week antibiotic run-in; 391 were included in the primary analysis.9 On the primary outcome, HbA1c, berberine alone and berberine plus probiotics both beat placebo by a similar margin (−0.99% and −1.04% versus −0.59%), and the combination was not significantly better than berberine alone (P=0.70).9 The probiotic by itself did no better than placebo. Insulin resistance (HOMA-IR, a secondary measure) improved with the combination but not with berberine alone.9 Both berberine arms had more digestive side effects. The trial was funded by Chinese government research grants; two companies donated the berberine, probiotics, and placebos, and the authors declared no competing interests.

In PREMOTE, berberine drove the blood sugar result, and adding that particular probiotic mix did not improve HbA1c further. For a combination product, that points to giving each ingredient a defined job. The probiotic should be chosen for its own goal, such as body composition or digestive comfort. The berberine form and dose should match what has been studied.

In our formula, B420 was chosen for a body composition role. In its main six-month trial of 225 adults with overweight or obesity, the pre-specified primary analysis (body fat change across all four groups) was not significant.10 Among participants who followed the protocol, B420 plus fiber showed 4.5% less body fat mass than placebo in an exploratory comparison, B420 alone did not reach significance, and a post-hoc analysis pooling both B420 arms showed a 2.4 cm smaller waist.10 DuPont Nutrition & Health, which developed B420 at the time, fully funded the trial, several authors were DuPont employees, and the sponsor helped draft the protocol and interpret the data.10 The lemon extract adds a separate metabolic signal: in a crossover trial in adults with high blood sugar, fasting glucose (the primary outcome) fell 5% and GLP-1 (a secondary measure) rose 17% from the start to the end of the 12-week period on 200 mg a day of an eriocitrin-standardized lemon extract (Eriomin).11 Those are within-period changes, not differences reported against placebo. Of 45 people randomized, 30 completed both periods. The ingredient's supplier funded the study and provided the extract; the paper declares no author conflicts, though the supplier has named the lead author as its chief scientific advisor.14

Berberine HCl vs. Dihydroberberine in These Formulas

The table splits evenly. Florajen, FOLONA, and PLZ use standard berberine HCl. WonderBiotics, DietWorks, and Toniiq use dihydroberberine.

Dihydroberberine was developed to get around berberine's poor absorption. In a randomized crossover pilot in five healthy young men, four doses of 100 mg DHB produced higher plasma berberine over the two hours after the last dose than four doses of 500 mg standard berberine.12 Glucose and insulin did not differ between conditions in that short pilot, and the study was funded by NNB Nutrition, which makes the DHB ingredient; one author was a paid adviser to the company.12 So the absorption advantage has human support, and the clinical outcome advantage is still being studied.

Read the milligram column with that in mind. A 100 mg dose of berberine HCl is small next to the 1.2 g a day used in PREMOTE.9 The DHB products use 100 to 200 mg per serving, the same per-dose amounts the absorption pilot tested, although that pilot gave four doses over about a day and did not test long-term daily use.12 Comparing milligrams across the two forms will mislead you, because they are absorbed differently.

What Do Users Say About Probiotic-Berberine Combinations?

Customer ratings and written reviews describe individual experiences with these products. They can help with capsule size, digestive tolerance, and purchasing practicalities, but they cannot establish whether the formula caused a weight change. Ratings also vary by platform and change as new reviews arrive:

  • WonderBiotics Probiotics for Weight Management has a roughly 4.8-star rating across more than 730 customer reviews.1 The visible reviews include people reporting 10 or 15 pounds lost and people reporting no change after four months.
  • Florajen Menopause Probiotic shows 4.2 stars from 85 ratings on Amazon.2
  • FOLONA shows 4.4 stars from 157 ratings on Amazon, where the AI-generated review summary highlights digestion and reduced bloating.3
  • DietWorks GLP-1 Companion shows about 3.7 stars from roughly 175 ratings on Amazon.4

Star averages flatten everything into one number, so read the reviews for these signals instead:

1. How long the person took it. Weight and waistline reviews written in the first week tell you little. Look for people past the two-to-three-month mark.
2. Early digestive changes. Berberine and new probiotics can both cause gas or loose stools at first. Note whether reviewers say it settled after a week or two or kept going.
3. What else changed. A review that mentions a new diet, a GLP-1 prescription, or a workout plan is describing the whole routine, not the capsule alone.
4. Patterns in the low ratings. One-star reviews that repeat the same complaint (taste, stomach upset, shipping, subscription billing) are more informative than scattered ones.

One Daily Capsule With Named Strains and DHB

If you want one capsule that covers a strain studied for body composition, a better-absorbed berberine form, and a second metabolic ingredient, WonderBiotics Probiotics for Weight Management was built for that. It combines B420 and seven other coded strains, dihydroberberine, and eriocitrin-standardized lemon extract, and you take it once a day after a meal.1 We suggest giving it three to six months alongside your eating and activity routine; for reference, the main B420 trial ran for six months.10

It is a dietary supplement that supports your routine; it does not treat diabetes, obesity, or any other condition, and it does not replace GLP-1 medication. Berberine can change how the liver processes some prescription drugs,13 so talk with your doctor or pharmacist before starting any berberine-containing product, especially if you take diabetes medication or any other prescription. Do not stop or change a prescribed medication on your own.
