Bloating, Gas, and Regularity Show Up Separately in Trial Data
A 2024 placebo-controlled trial of Bifidobacterium animalis subsp. lactis HN019 in 229 adults with functional constipation shows how differently these outcomes can behave inside the same study. Researchers pre-specified a testing order: check the primary outcome first, weekly complete spontaneous bowel movements, and only move on to the next outcome in line, straining, then bloating, then overall symptom scores, if the one before it hit statistical significance.1 Bowel movement frequency, the trial's primary target, didn't improve enough with HN019 to count as a statistically meaningful difference from placebo, so the formal testing sequence stopped there.1 Participants taking HN019 did report improved bloating scores compared with the placebo group, but because bowel frequency was the trial's primary design target, that bloating result is a secondary, less certain finding rather than the study's confirmed outcome.1

We built WonderBiotics Probiotics for Gut Health around HN019 because, among the trials reviewed here, it's the only strain with a trial where whole-gut transit time was the primary, pre-specified outcome. In that trial, the high-dose HN019 group cleared test markers significantly faster than both the low-dose and placebo groups by day 14 (Kaplan-Meier comparison, P<0.05); separately, the three-group difference in WGTT change from baseline reached significance by ANOVA (P<.001), and a baseline-adjusted analysis of day-14 WGTT across the three groups was also significant (P=.001).2 A second, larger trial in 228 adults with diagnosed functional constipation found no significant difference on the primary outcome, colonic transit time, or on overall bowel frequency; a post-hoc subgroup of 65 participants with three or fewer bowel movements per week during a 14-day run-in period did show higher frequency when the two HN019 dose groups were pooled and compared with placebo (P=.01).3

Slow, sluggish transit is a common driver of bloating, since gas and stool both build up when things move too slowly through the colon. That's the mechanism our formula targets first: speeding up whole-gut transit is a foundational step toward easing bloating that's tied to slow motility, even though direct evidence on bloating itself, discussed above,1 is still accumulating.
Align Probiotic Bloating Relief + Food Digestion: The Longest Trial History, With a Dose Caveat
Align's core strain, B. longum 35624 (studied for years under the older name B. infantis 35624), has multiple published trials addressing bloating and IBS-type symptoms, with an important dose-and-formulation caveat. In a 362-woman IBS trial, only the middle dose (10⁸ CFU) beat placebo, on the primary endpoint and on symptom scores that specifically included bloating, bowel dysfunction, incomplete evacuation, straining, and gas; the lowest dose (10⁶ CFU) didn't separate from placebo, and the highest dose (10¹⁰ CFU) had documented formulation problems.5 A separate 77-person IBS trial using 10¹⁰ CFU (a different formulation than the one that failed in the trial above) found the strain outperformed placebo on abdominal pain, bloating, and difficult defecation across most treatment weeks.6 In a larger general-population trial (302 randomized, 275 evaluable, not an IBS-diagnosed sample), overall symptom severity didn't separate from placebo, but the number of bloating-free days did (P<.05).7 A newer open-label study with no placebo arm enrolled 37 people, with 33 in the overall per-protocol population; the IBS symptom severity score analysis, reported for 31 of those participants, showed an 82-point drop over 8 weeks (P<.001), a number that a study without a control arm can't separate from placebo response or regression to the mean.8
Align now sells this strain in a capsule built for this exact symptom set. The label lists 10mg of the 35624 strain (1 billion CFU at manufacture, 10 million CFU guaranteed through the best-by date), a 23mg DIGEZYME multi-enzyme blend (alpha-amylase, neutral protease, lactase, cellulase, lipase, the same five enzyme classes WonderBiotics uses), and 2.4mcg of vitamin B12.9 Align doesn't list a price for this SKU on its own site. None of the trials cited here enrolled a diagnosed-constipation population, so regularity wasn't what they measured.
Culturelle Digestive Daily: A Single-Strain Formula for Gas and Travel
Culturelle's current label is straightforward: 40mg of LGG (10 billion CFU) and 200mg of inulin from chicory root, nothing else.10 The current label claims are just as narrow: "reduce occasional digestive upset including gas, bloating and diarrhea" and help with travel-related digestive issues, with the dose doubling to twice daily before and during trips.10 Constipation isn't on that list. At $17.99 for 30 capsules ($0.60/day), it's the cheapest option here.
Physician's Choice 60 Billion Probiotic: Ten Species, No Strain Codes on the Label
This product carries the broadest label claim on this list: "supports occasional constipation, diarrhea, gas & bloating," backed by 60 billion CFU across 10 species and a 150mg prebiotic blend of organic inulin (from Jerusalem artichoke and chicory) plus acacia fiber.11 We pulled the label directly from the NIH's Dietary Supplement Label Database to confirm the wording, and it lists species only: Lacticaseibacillus casei, Lactobacillus acidophilus, Lacticaseibacillus paracasei, Ligilactobacillus salivarius, Lactiplantibacillus plantarum, Bifidobacterium lactis, B. bifidum, B. longum, B. breve, and Lactobacillus bulgaricus, with no strain codes attached.12 Without a strain code, there's no way to trace this product's "60 billion CFU, 10 species" claim to a published study the way HN019 or 35624 can be traced. Physicianschoice.com listed this product at $41.27 for a one-time 30-count purchase when we checked, though pricing on the site has varied across visits, so treat that number as an example rather than a fixed price.
Physician's Choice Bloat + Gas Probiotic: A Named Strain at a Lower CFU Dose
This product's CFU count is the lowest on this list, 2 billion, and it names its strain: Bacillus subtilis OPTIBIOME BS50, combined with tributyrin (branded CoreBiome, a postbiotic ingredient that serves as a source of butyrate, a short-chain fatty acid) and organic acacia fiber in a 325mg blend, plus a 50mg herbal blend (artichoke leaf, ginger root) and a 36mg enzyme blend (lactase, alpha-galactosidase), one capsule daily for $47.03 per 30-count bottle.13
BIO-CAT, the manufacturer that holds the OPTIBIOME BS50 trademark, funded a 76-person, six-week trial of the same strain at the same 2-billion-CFU dose. The trial's pre-specified composite outcome combined bloating, burping, and flatulence into a single responder measure: 47.4% of the BS50 group improved on that composite versus 22.2% on placebo (P=.024; adjusted OR 3.2, 95% CI 1.1-8.7, P=.026). An individual-symptom analysis of burping found a similar pattern (44.7% vs. 22.2%, P=.041).14 BIO-CAT funded the trial, employed two of the authors, and has a pending patent on the finding, so the effect size should be read as industry-generated rather than independently replicated.
Alpha-galactosidase, the enzyme in this product's 36mg blend (the amount specific to alpha-galactosidase itself isn't disclosed), is sold on its own as Beano and breaks down the raffinose-family oligosaccharides in beans and cruciferous vegetables that cause gas. In a small 1994 crossover trial, 19 adults ate an identical high-fiber meal twice, once with the enzyme and once with placebo; flatulence events per hour were lower with the enzyme across six hours of follow-up (P=.016), though the difference reached significance at only one time point, and bloating and abdominal pain scores didn't differ between groups.15 That trial tested the enzyme class in general, not this product's specific undisclosed alpha-galactosidase amount. It also measured gas after specific foods rather than bloating as a sensation.
Matching a Product to Your Pattern
- Infrequent, hard-to-predict bowel movements: among the trials reviewed here, HN019 is the only strain with a transit-time trial where that was the pre-specified primary outcome, though a second HN019 trial missed on the same outcome in a different population.1,2
- Gas that shows up predictably after beans, cruciferous vegetables, or other raffinose-rich foods: alpha-galactosidase targets that specific mechanism, separate from any probiotic strain's effect.15
- General bloating and discomfort without a clear food trigger: 35624 has multiple published trials on this symptom cluster, with positive findings at 10⁸ CFU in one trial and 10¹⁰ CFU (a different formulation) in another.5,6,7
- Occasional gas or looseness tied to travel or diet changes, no constipation: Culturelle's single-strain formula matches its current label claims.10
- Burping specifically, alongside bloating and gas: the BS50 trial's composite endpoint covered exactly that symptom cluster, at the same 2-billion-CFU dose sold in the Physician's Choice Bloat + Gas capsule.14
The Adjustment Period, and When to Stop
Some people report temporary gas or bloating after starting a new probiotic, including ours; timing and duration vary by person and by product. If new symptoms feel worse than what you started with rather than milder, that's a reason to stop and check in with a doctor, not push through.
When to See a Doctor About Bloating
Bloating that's new, persistent, or paired with other symptoms deserves medical evaluation before you try another supplement. See a doctor if bloating comes with abdominal pain, blood in the stool or dark tarry stools, diarrhea, worsening heartburn, vomiting, or unexplained weight loss.16 The American Cancer Society also notes that bloating, pelvic or abdominal pain, feeling full quickly, and urinary urgency are common early ovarian cancer symptoms in women, and recommends seeing a doctor if any of these happen more than 12 times a month.17 No probiotic, fiber supplement, or enzyme blend substitutes for that evaluation.
